Heart Failure Bridge Clinic

Congestive heart failure (CHF) is the top reason for hospital admissions and readmissions for people age 65 and older. With that in mind, Windham Hospital has opened the Heart Failure Bridge Clinic to help successfully transition CHF patients from inpatient care to outpatient and self-care. Once a patient is referred to the program there is an initial assessment followed by a free 90-minute one-on-one session with a nurse with education about medication, healthy eating, and weight and symptom monitoring. The goal: keep patients out of the hospital and empower them to manager their condition.

A Growing, Regional Model of Care for Cardiac Patients

Hartford HealthCare Cardiology, the practice that serves Windham Hospital cardiac patients, moved to a regional model in 2015 adding providers to its growing Windham and Norwich practice. Windham Hospital is working hard to keep more care in the community, but it’s affiliation with Hartford HealthCare allows patients with more advanced needs like transplant and bypass surgery to be seamlessly connected to tertiary care at Hartford Hospital.
